A stolen passports requires following a list of legal procedures including the reporting of the theft and an application for a new one. The State Department must be notified of your loss immediately and completion of the DS-64 form should be posted.
Accuracy
Ensure that every item has been filled in and all information is correct as this will prevent delays in the processing of the particular forms. Be sure to contact local authorities in determining which forms should be submitted as often a police report will have to be completed with application forms in submission. The report will include details surrounding the theft and should be mailed with relevant documentation to the U. S. Passport Services.
Loss of a Valid Passport
If your legal document was valid at the time that it went missing, the DS-11 and DS-64 forms will have to be completed and submitted together. This is because one will have to apply for a new legal document, which is the reason that the DS-11 form is needed to be completed and delivered to relevant authorities. A number of specific documents are necessary when applying to replace the important travel document.
Identification
These documents include presentation of a certified birth certificate or Certificate of Naturalization. For proof of identification one may include copies of driver license and government or state issued identification card including 2 passport size pictures. Charges apply to the application for a new document and if the travel item is needed in a short period of time, extra fees apply to expedited damaged pasport services.
